What is TailorCare?
TailorCare is a care navigation program that provides deeply personal, evidence-based support to people with joint, back, and muscle pain. Our care team includes licensed physical therapists to help you navigate your treatment options, connect with healthcare specialists, and choose a custom care plan.
How does TailorCare work?
TailorCare listens to your concerns regarding your joint, back, or muscle pain. A Clinical Navigator, who is a licensed physical therapist, will spend time getting to know you through a comprehensive assessment.

Once your assessment has been completed, your Clinical Navigator will help you navigate your treatment options, connect you with in-network healthcare specialists, and schedule your first appointment. Throughout your treatment, your Clinical Navigator will follow up with you when you decide on your care pathway and at the end.

What joint, back, or muscle issues can TailorCare help with?
Back or lower back pain, neck, shoulder, knee, hip pain, ankle, elbow, wrist, and immobility.
How often will I hear from TailorCare?
The TailorCare team will follow up with you throughout your care journey to make sure you are feeling better and improving. If you are not progressing appropriately, we will help you choose a new care plan or help you overcome any barriers to care.
Will my primary care physician (PCP) be updated on my progress?
The TailorCare team will send a note to your primary care physician when you agree to a care pathway and then one at the conclusion of care.
Is TailorCare covered by my insurance?
TailorCare services are available through some insurance plans. Contact us to see if your plan is covered. However, you are still responsible for any copay/coinsurance associated with physical therapy, occupational therapy or visits with a specialist.
