What conditions can TailorCare help with?
Back and lower back pain, neck pain, shoulder pain, knee pain, hip pain, ankle pain, elbow pain, wrist pain, and issues with mobility and movement. Recent injury or ongoing condition — TailorCare can help.
Is there a cost to me?
TailorCare services are available to eligible Humana members and offered through your Humana Medicare Advantage plan at no additional cost. No referral needed. You are responsible for any copays and/or coinsurance for visits to your physician, PT, or specialist.
How do I get started?

How does TailorCare work?
Will my PCP be updated on my progress?
Yes. TailorCare will send a note to your primary care physician when you agree on a path forward and again at the conclusion of your care.
